A healthcare power of attorney lets you choose the person (called your healthcare agent) who will speak for you and make healthcare decisions (and for your children) if you are ever unable to do so yourself without needing court involvement or relying on the state’s default priority rules. You are also able to decide the types of powers your healthcare agent has when making those decisions.

This document can also allow your agent to communicate your wishes regarding life support and may include your wishes regarding organ donation and the disposition of your body.
